# 45. Customer Vehicle Data Sheet

The [Customer Vehicle Data Sheet](https://autosoftware.company/mpdf/customer_vehicle_data_sheet_pdf.php) is not a report but a printable intake form. Clicking it in the Reports menu opens a one-page PDF headed CUSTOMER & VEHICLE DATA SHEET (CUSTOMER TO COMPLETE) with the shop's name and contact details at the top and blank lines for the customer to fill in by hand: the vehicle, the customer's own details, the reason for the visit and when the age and mileage related services were last done. It is always blank; nothing from ARM is filled in. Use it at the counter for a new customer or a walk-in, so the details and the service history you cannot see on the car are written down before the write-up, and keep a stack printed for busy mornings.

## How do I print the Customer Vehicle Data Sheet?

1. From the Home Screen, click Reports → Management & Operational Reports → Customer Vehicle Data Sheet. The PDF opens in a new tab and the browser's print dialog opens with it.
2. Choose the printer and the number of copies and print. Close the tab to return to the Reports page.
3. Hand the sheet to the customer with a pen, and copy the answers into the customer, vehicle and invoice screens when the write-up is done.

## About this screen

The sheet has four sections. I. VEHICLE DATA asks for LICENSE PLATE #, ODOMETER READING, VIN NUMBER, YEAR, MAKE and MODEL. II. CUSTOMER DATA asks for FIRST NAME, LAST NAME, STREET ADDRESS, CITY, ZIP CODE, EMAIL ADDRESS, BEST TELEPHONE TO REACH YOU and HOW DID YOU FIND US?, the last of which feeds the referral source on the invoice. III. REASON FOR YOUR VISIT TODAY (MARK ALL APPLICABLE) has tick boxes for OIL & FILTER SERVICE, SMOG INSPECTION, A/C ISSUES, BRAKE SYSTEM ISSUES and OTHER REASON (PLEASE WRITE). IV. DATA FOR AGE & MILEAGE RELATED SERVICES explains that some maintenance items cannot be judged by looking at the car and asks the customer to say, under How Many Miles Ago? and How Many Months Ago?, when each of eleven services was last performed: Lube, Oil & Filter, 30k/60k/90k/120k Service-4CYL, Transmission Drain & Refill, Transmission Oil, Filter & Gasket, Transmission Fluid Flush, Diff Fluid - Drain & Refill, Brake Fluid Service, Radiator Fluid, Fuel Injection Cleaning, Fuel Filter and Spark Plug Replacement. The list of services is fixed and cannot be changed from ARM. The tick boxes are printed squares for a pen, not boxes that can be ticked on screen.

The sheet can be saved under Favorites with the heart icon beside its name on the Reports page. Which reports you see depends on your admin group permissions.
